Only when the string ACCOUNTS SUMMARY REPORT is found on line 3 in the Column range 20 - 45 and the string DALLAS OFFICE is found in line 4 or 5 in column range 1 - 16, will the page be assigned to recipient BR12.
Note that the second WHEN identification searches over two lines because the identifying string can be produced on line 4 or 5.
